COVID-19 Hastalarında Yeni Bir Yaklaşım Olarak Oksihemoglobin Karboksihemoglobin, Kan Gazı Değerlerinin İncelenmesi: Longitudinal Bir Çalışma

Abstract

Amaç: Bu çalışmada COVID-19 tanısı koyulan ve yoğun bakım ünitesinde (YBÜ) yatarak tedavi gören hastaların rutin kan alımı sonucunda karboksihemoglobin, oksihemoglobin, pCO2 ve pO2 parametrelerinin retrospektif olarak değerlendirilmesi amaçlanmıştır.
Materyal ve Metod: Çalışma Ankara Şehit Sait Ertürk Devlet Hastanesi’ne COVID-19 şüphesi ile başvuran ve ardından korona virüs enfeksiuonu koyularak YBÜ’ne yatışı gerçekleştirilen 2000 hasta ile gerçekleştirilmiştir. Ek olarak cinsiyet ayrımı yapılmaksızın 300 korona virüs enfeksiyonlu kişi ve 300 sağlıklı gönüllü ile çalışma sürdürülmüştür. Veriler YBÜ’ne yatış sürecine kadar gerçekleştirilen rutin kan alım işlemi sonuçlarıdır.
Bulgular: PCR sonucu pozitif olan hastaların pH'ı, negatif test sonucu olanlara göre önemli ölçüde daha düşüktür. Çalışma evreninde pCO2 değerleri göz önünde bulundurulduğunda %61,6 oranındaki hastalar referans değerinin dışında yer almaktadır. Kadın ve erkek hastalar arasında oksihemoglobin parametresine göre anlamlı bir fark bulunmuştur [t = -10.643, p<0,001]. Sağlıklı bireyler ve hastalar arasında pO2 değerleri arasında anlamlı bir fark bulunmuştur [t = 12016, p<0,001]. Ancak sağlıklı bireyler ve hastalar arasında pCO2 değerleri arasında anlamlı bir fark bulunamamıştır [t = -0,416, p>0,05], ayrıca sağlıklı bireyler ve hastalar arasında oksihemoglobin değerleri arasında anlamlı bir fark bulunamamıştır [t = 1725, p>0,05]. Hastaların pO2 ve pCO2 düzeyleri arasında negatif yönde orta düzeyde anlamlı bir ilişki bulunmaktadır (r=-0,443, p<0,001).
Sonuç: Kan gazı değerleri üzerinde yapılan retrospektif çalışmada pCO2 değeri artış gösterirken pO2 değeri düşmüştür. Buna göre hastaların CO2’ü substrat olarak kullanan CA enzim aktivitelerinin bu tür enfeksiyonlarda önemli bir kriter olabileceğini düşündürmektedir. CA enzimi CO2 artışı ile doğru orantılı artan bir enzimdir. Burada incelenen sonuçlar göz önünde bulundurulursa ekspresyonunun arttığı öngörülmektedir.
