Abstract
Çocuklarda akut başlangıçlı trombositopeninin en sık nedeni immün
trombositopenik purpuradır (ITP). Brusellada görülen hematolojik komplikasyonlar
hafif anemi, lökopeni, bazende ciddi trombositopeni ile seyredebilir. Brusella
ve ITP birlikteliği nadir olarak görülmektedir. Biz burada diş eti kanaması
şikayetiyle başvuran olguda brusellaya sekonder ciddi ITP geliştiğini
vurgulamaya çalıştık.
Daha
önceden bilinen bir hastalığı olmayan 15 yaşında erkek hasta çocuk acil
polikliniğine dişeti kanaması şikayetiyle başvurdu. Fizik muayenede sağ üst
molar diş etrafından aktif kanaması, tüm vücutta peteşi, purpura mevcuttu. Hastanın, hepatosplenomegali ve lenfadenopatisi
yoktu. Tetkiklerinde beyaz küre sayısı: 8140 /mm3, HGB :11,7 g/dL HCT:35,6 trombosit sayısı: 2000 /mm3, AST: 104 U/L, ALT : 167 U/L ve Direkt Coombs testi negatif olan
hastanın kemik iliği
aspirasyonu ITP ile
uyumlu olarak değerlendirildi. Hastanın
trombositopeni etyolojisine yönelik yapılan tetkiklerinde brucella pozitif
saptandı. Hastaya brusella tedavisi başlandı. Trombosit aferez süspansiyonu
almasına rağmen trombosit sayısı yükselmemesi nedeniyle hastanın
trombositopenisi brusellaya bağlı ITP şeklinde yorumlandı ve intravenous immunoglobulin (IVIG) tedavisi verildi. Brusella ve IVIG tedavisinin 3. gününde
sonra trombosit sayısı yükseldi ve karaciğer enzimleri düştü.
Brucella
infeksiyonu sistemik otoimmün yanıtı tetikler ve buna bağlı olarak da otoimmun
trombositopeni gelişir. Brusella trombositopeninin bilinen nedenleri arasında
olup, bizim sunmuş olduğumuz olguda trombosit aferez süspansiyonları ile
trombositopeninin düzelmemesi ve IVIG sonrası anlamlı yanıt alınması
brucelloza bağlı olarak gelişen immun trombositopeniyi destekler niteliktedir.
