COVİD-19 İLE İLİŞKİLİ KANDİDEMİ

Abstract

Kandidemi, yüksek ölüm oranları ile dünya çapında sık görülen bir kan dolaşımı enfeksiyonudur. Hastalık, geniş spektrumlu antibiyotiklere, kortikosteroidlere ve diğer immünosupresif ajanlara maruz kalma, intravasküler kateterler, mekanik ventilasyon ve diyaliz gibi invaziv prosedürler dahil olmak üzere çeşitli kandidemi risk faktörleri ile özellikle yoğun bakım gerektiren ciddi rahatsızlıkları olan hastaları etkilemektedir. COVID-19 hastalarının önemli bir kısmında akut solunum sıkıntısı sendromu (ARDS) gelişir. Bu durum YBÜ'sine yatış ve mekanik ventilasyon gerektirir, bu da hastaları bakteriyel ve fungal enfeksiyonlara bağlı nozokomiyal enfeksiyonlara yatkın hale getirir. İkincil enfeksiyonlu COVID-19 hastalarının yükünü ve bunların etiyolojik ajanlarını anlamak, COVID-19 hastalarının optimal yönetimi için çok önemlidir. COVID-19 hastaları arasında, Candida'ya bağlı süper enfeksiyon insidansı şu anda bilinmemektedir. Candida süper enfeksiyonunun artmış mortaliteye yol açıp açmadığı veya yalnızca COVID-19 enfeksiyonunun ciddiyetinin bir göstergesi olup olmadığı da bilinmemektedir. Yüksek mortalite göz önüne alındığında, kandideminin erken tanınması ve uygun antifungal tedavi ihtiyacı, YBÜ'deki COVID-19 hastalarının sonuçlarını iyileştirmek için temel gerekliliklerdir. Bu yazıda COVİD-19 hastası olan ve Candida süperenfeksiyonu görülen 2 olgumuzu sunmayı amaçladık. Bu raporla, COVID-19 hastalarıyla ilişkili nozokomiyal enfeksiyonları tanımaya ,klinisyenlerin dikkatini çekmeyi hedefliyoruz.
