Total Diz Artroplastisi Sonrası Postoperatif Erken Dönemde Kinezyo Bantlama Tedavisi Alan Hastalarda Postoperatif Üçüncü Aydaki Fonksiyonel Performansın Değerlendirilmesi
Abstract
AMAÇ:
Total diz artroplasitisi (TDA) sonrasında hastaların fonksiyonel kapasiteleri sağlıklı popülasyondan daha düşüktür. Bunun nedeni hareket açıklığının ve kas kuvvetinin post operatif erken dönemde kazanılamamasıdır. Bu çalışmada amaç TDA sonrası erken dönemde ilgili alt ekstremitesine kinezyo bantlama yapılan hastaların postoperatif üçüncü aydaki fonksiyonel performansının değerlendirilmesidir.
GEREÇ VE YÖNTEM:
Çalışmaya dokuz TDA hastası (çalışma grubu) ve alt ekstremitelerine yönelik herhangi bir şikayeti ya da cerrahi öyküsü olmayan dokuz sağlıklı olgu (kontrol grubu) olmak üzere on sekiz kişi katıldı. Çalışma grubunda yedi kadın, 2 erkek olgu; kontrol grubunda sekiz kadın, bir erkek bulunmaktaydı. Çalışma grubunun yaş ortalaması 61,33±7,40; vücut kitle indeksi ortalaması 31,09±2,95 iken, kontrol grubunda bu değerler sırasıyla 64,44±4,72 ve 31,52±4,49 idi. Çalışma grubu postoperatif ikinci ve on beşinci günler arasında ödemi azaltmaya yönelik kinezyo bantlama tedavisine alındı. Post operatif üçüncü ayda kontrole gelen hastaların diz fonksiyonları Lysholm diz skoru, fonksiyonel performansları ise altı dakika yürüme testiyle değerlendirildi. Kontrol grubuna da aynı değerlendirmeler yapılarak elde edilen sonuçlar SPSS 16 istatistik paketi kullanılarak analiz edildi.
SONUÇLAR:
Çalışma ve kontrol gruplarında Lysholm skorlarının benzer olduğu, altı dakika yürüme testi skorunun çalışma grubunda daha yüksek olduğu görüldü (p=0,004). Çalışma grubunda Lysholm skoru arttıkça altı dakika yürüme testi skorunun da arttığı (p=0,03), kontrol grubundaysa değişkenler arasında böyle bir ilişki olmadığı görüldü.
TARTIŞMA:
TDA uygulanan hastalarda eski fonksiyonel durumlarından daha iyi bir duruma dönmesi için çeşitli post operatif modaliteler tanımlanmıştır. Erken post operatif dönemde kinezyo bantlama uygulaması TDA hastalarının fonksiyonel kapasitelerini artıran bir yaklaşımdır. Bantlama tedavisi almayan TDA olgularının da benzer şekilde değerlendirileceği çalışmalar gerekmektedir.
